The Sary Tash – Karamyk road, EM-03, runs east to west for 136 km. It forms part of an old Silk Road connecting Kashgar in China with Dushanbe in Tajikistan and the Afghanistan and Iran to the south. Sary Tash is located at the main junction of the Osh- Irkeshtam-Kashgar (China) road and the Pamir highway. Karamyk is located on the Kyrgyz side of the border with Tajikistan.

The existing road is a lightly trafficked category III-V road with a single carriageway. The first 24 km west of Sary Tash are asphalt paved but in poor condition, and similarly through the small towns of Sary Mogol (3 km) and Daroot Korgon (4 km). The remainder is a gravel road also in poor condition with the last 2 km up to Karamyk Pass consisting of a narrow earthen track.

It is planned to upgrade the road to a category IV road to form part of the strategic transit road between China and the West with a substantial increase in traffic from new mining operations in the project area from transit traffic. This will require rehabilitation of the road including improvements to horizontal and vertical alignments, drainage, pavement, some bridges and most notably scour, erosion and river protection works as the road is under threat of washouts each snow melt season.
